Horizon Scanning: FDA panel supports extension to use of duloxetine (Cymbalta)
Source: Reuters Area: News Reuters reports that a United States Food and Drug Administration (US FDA) advisory panel has supported an extension to the licensed indication for duloxetine (Cymbalta?) to include chronic back pain. The panel voted whether it effectively treated lower-back pain and osteoarthritis and voted 8-5 to approve its use for back pain, however, by a 4-9 vote they found there was insufficient data to support its use in osteoarthritis. The panellists also said there was not enough data to support a stronger 120mg dose over the current 60mg version.   The FDA will consider the advisory panel’s recommendation before making its final approval decision. Physical conditioning programs for improving work outcomes among workers with back pain.
CONCLUSIONS: The effectiveness of physical conditioning programs in reducing sick leave for workers with back pain remains uncertain. For acute back pain, these programs probably have no effect; for subacute back pain, the effect is unclear, and for chronic back pain there is a small effect at one year follow-up that does not last in the long run. Remaining heterogeneity could not be explained by meta-regression. A better understanding of the mechanism behind physical conditioning programs and return to work is needed to develop more effective interventions. Chronic back pain is among the most common patient complaints. Its prevalence and impact have spawned a rapidly expanding range of tests and treatments. Some of these have become widely used for indications that are not well validated, leading to uncertainty about efficacy and safety, increasing complication rates, and marketing abuses. Recent studies document a 629% increase in Medicare expenditures for epidural steroid injections; a 423% increase in expenditures for opioids for back pain; a 307% increase in the number of lumbar magnetic resonance images among Medicare beneficiaries; and a 220% increase in spinal fusion surgery rates. The limited studies available suggest that these increases have not been accompanied by population-level improvements in patient outcomes or disability rate…
